It has been reported that the "Melrose Place" alum has been staying in an in-patient facility for two weeks amid fears of her well-being. According to Access, which first reported the news, Heather "doesn't seem to be making any kind of progress."

The publication also suggests that Heather "continues to drink and abuse people around her." Meanwhile, Radar Online reports that her return to rehab was sparked by a nasty fight with on-and-off boyfriend Chris Heisser. "Heather and Chris got into a screaming match, and after they finally cooled down she realized she'd lost control again and decided she needed to head back to rehab," a source tells the gossip site.

While it remains unclear what is the real reason behind her latest rehab stint, it is said that this issue is taking a toll on her family, especially her 21-year-old daughter Ava, who allegedly is in a "horrible" state due to this. "Her issue is alcohol, pills and her mental health. She doesn't want anybody to hold her accountable," a second insider says to PEOPLE. "She has no job to keep her accountable. This has been tearing up her family."

Her family allegedly tries to be as hopeful as they can, but they are concerned because Heather has sought professional help numerous times. "It's just hard to tell," the source goes on saying. "She's not going into anything new is the problem though; she's just going back to something she has done before. She has been to rehab more than 20 times, perhaps 25 times."

"She won't take any help from anybody close to her who wants to help her. She has a 5250 [hold]. And nobody did a conservatorship, nobody stepped up. ...It's the same mental health issues as well," the insider adds. "Somebody has to take a lead on this, and nobody has any rights here except her parents."

Heather was previously placed on a 5150 hold after her therapist determined that she was experiencing mental breakdown. She left her rehab facility in December last year.
